Am I reading this wrong… Or does the article say this is an issue because people are trying to refill the alcohol while the fire is lit?

Unless that was somehow stated as a feature that isn’t working correctly I don’t see how this isn’t just incompetence and lack of common sense.
Normally I’d agree, however they mentioned alcohol fires can be invisible, so I think it’s a design flaw in this case. That said, I may be wrong, as my wife is in charge of Fire in our marriage.
I think that was only part of it. The other part is this:

Yet another VC dude bro combining two incompatible terms and wondering what went wrong.
The problem was how you refilled it. If it had a different refilling mechanism that could be done safely, even if lit, it’d probably be okay.
We light things on fire ontop of other things all the time.
edit: Oh and some form of indicator if there’s still a fire, given the invisible flames. Even though the issues were with refilling, you know someone will eventually still put their hand over/into it.
